Social prescribing is a way of supporting you to take care of your own health and wellbeing.

Our Social Prescribing Link Workers will get you connecting with the local services, activities, groups etc that will get you where you want to be.

We listen, focus on what matters to you and help you create a personalised plan for your own health and wellbeing.

Tell us your goal, we’ll work together to make a clear and simple plan.

Health Coaching

Health Coaches work with you to build your confidence, knowledge and skills to take ownership of your health and wellbeing.

We support you to make informed choices that are meaningful to you.

You may be referred to our services by a professional e.g. a health care worker, like a GP or physio, or by contacting us yourself. We will get back to you by phone/text and book in a chat with a link worker.

Heeley Trust provides the social prescribing service for Heeley Plus Primary Care Network, working alongside the practice staff, physios, pharmacists and others to make sure the support you’re getting is right for you. We help the network to run support groups for long term conditions and run our own health and wellbeing groups and social activities.
